Measles (Rubeola)



Definition

Measles is a highly contagious viral infection caused by the Measles virus (a paramyxovirus of the genus Morbillivirus), characterized by fever, cough, conjunctivitis, and a distinctive erythematous maculopapular rash.


Etiology / Causes

  • Caused by Measles virus.

  • Transmission occurs via respiratory droplets from coughing or sneezing.

  • Incubation period: 10–14 days.

  • Risk factors: Unvaccinated children, immunocompromised individuals, malnutrition.


Signs and Symptoms / Clinical Features

  • Fever (often high, around 104°F or 40°C).

  • Cough, coryza (runny nose), and conjunctivitis – the 3 C’s.

  • Koplik's spots – small white spots on the buccal mucosa.

  • Maculopapular rash starting on the face and behind ears, spreading to trunk and limbs.

  • General malaise, irritability, and photophobia.


Diagnosis

  • Clinical diagnosis based on symptoms and rash.

  • Confirmatory tests:

    • Measles-specific IgM antibodies in blood.

    • RT-PCR for measles RNA.


Complications

  • Otitis media

  • Pneumonia

  • Diarrhea and dehydration

  • Encephalitis

  • Subacute sclerosing panencephalitis (SSPE)

  • Death in severe cases or malnourished children


Treatment (Conventional Approach)

  • No specific antiviral treatment.

  • Supportive care:

    • Antipyretics (for fever)

    • Fluids and rest

    • Vitamin A supplementation (recommended to reduce severity and mortality)

    • Management of complications like pneumonia or diarrhea


Homeopathic Therapeutics

1. Pulsatilla

  • Indications:

    • Useful in mild cases of measles, especially in children.

    • Rash appears slowly or is suppressed.

    • Thick yellow nasal discharge, bland and profuse.

    • Patient is weepy, clingy, and wants to be carried.

    • Associated loose cough, low-grade fever, thirstlessness.

  • Modalities:

    • Worse in warm room; better in open air.

  • Clinical Note:

    • Useful when eruption does not develop fully or is suppressed due to medication or weather.


2. Morbillinum (Nosode)

  • Indications:

    • Used as a prophylactic in epidemic conditions.

    • Can also be used therapeutically if the disease does not follow its normal course.

    • Helpful when rash is delayed, retrocedent, or complications arise.

  • Clinical Note:

    • Particularly useful when symptoms are not matching other polychrest remedies, or in severe epidemics.


Prevention

  • Measles vaccine (part of MMR: Measles, Mumps, Rubella) at 9–12 months and 15–18 months.

  • Isolation of infected individuals.

  • Public health education and hygiene practice
